Some people are just rather good at certain things, and it makes others suspicious. I've always said that there are way too many people accused of hacking in CS... people say it's overrun with hackers, but that's bull... I've seen it on a few servers, but for the most part it's clean, just skilled people get accused nonstop.
People are most suspicious about headshots and shots through walls or around corners. For the first, there are some guns that people seem very skilled at getting headshots with... the biggest are the AK, scout, and colt, at least by my estimation. Mac-10 and deagle have a tendency too. They are all relatively powerful per hit, so one headshot and you're usually screwed. It makes it seem like headshots are more frequent... but someone using a weaker gun may hit you many times in the head and you don't remember because that's not how you died. Regardless, some guns seem drawn to heads, like the ak, and when you get several in a row, people accuse. It just happens, and sometimes it's just good aim.
For the next thing, I'll tell you what... it seems like no one has ever heard of using sound and wearing headphones. Footsteps can tell you exactly where someone is... and even if you walk, if you fall from far enough there is a sound (even off a couple of steps), or pick up something, or reload, sound is made. You can assess pretty well where the enemy is. On top of that, sometimes people think no one can see them. I was playing militia yesterday, and Ltrain got shot in the head by rush, and he was like "He could see my head through the rocks????" Now, he wasn't accusing, but I was watching, and just the tip of his head was showing, so he was seen. Guns can show your position, or if you slightly move, or if you take a peak... you can get spotted. But in hindsight, people seem to exclaim "no, I never moved, no way he could see me!!!" If you REALLY didn't move, and you couldn't be spotted, it may be because you're in a frequently camped spot, and he sends a few bullets through the box... again, you make a noise when you are hit, and if he hears that, he keeps firing until you die.
